Neuroimmune related antibody detection - AQP4 antibody (registration certificate number: National Medical Products Administration 20192400461)
1. Background
Neuromyelitis optica (NMO), also known as Devic's syndrome, is a neuroimmune disease mediated by an immune response that accumulates in the optic nerve and spinal cord. Neuromyelitis optica spectrum disorders (NMOSD), in addition to classic optic neuritis, also include non optic neuritis or myelitis, clinically manifested as optic neuritis or myelitis, including segmental transverse myelitis or isolated optic neuritis. There is a specific standard substance in the serum of NMO or NMOSD patients - immunoglobulin G autoantibody (NMO IgG). The main antigen of this autoantibody is aquaporin 4 (AQP4).
2. Clinical significance
Detecting AQP4 antibodies can be used for:
A: Optic neuromyelitis and is an auxiliary diagnosis for neuromyelitis spectrum diseases;
B: Diagnosis of demyelinating patients before treatment;
C: Follow up monitoring of patients with optic neuritis and myelitis spectrum;
D: Differential diagnosis between optic neuritis myelitis and optic neuritis myelitis spectrum diseases and multiple sclerosis (MS).
3. Detection principle
The neuroimmune related antibody detection AQP4 antibody detection kit adopts bridge enzyme-linked immunosorbent assay technology, which enables the antibody (AQP4 Ab) in the test sample to simultaneously bind to the antigen (AQP4) encapsulated in the micropores and the biotinylated antigen (AQP4 biotin). By detecting the bound AQP4 biotin through enzyme-linked immunosorbent assay, the concentration of AQP4 Ab in the test sample can be calculated.
4. Testing process
1) Add standard quality control samples;
2) Add dissolved AQP4 biotin;
3) Incubate at room temperature with shaking for two hours;
4) Wash the board
5) Add diluted enzyme conjugates;
6) Shake incubation
7) Wash the board
8) Add chromogenic substrate
9) Incubate in the dark
10) Add termination solution
11) Read the absorbance values using 450nm and 405nm wavelengths respectively
5. Characteristics
Sensitivity: 80%, in 83 patients with optic neuritis and myelitis; 100% of 64 patients with optic neuritis and myelitis spectrum diseases tested positive for NMO IgG in their serum.
Specificity: 99%, among 358 healthy individuals;
Standard range: 1.5-80 units/mL (RSR defined concentration);
Critical value: Negative:<3 units/mL; Positive: ≥ 3 units/mL;

It can be conducted in clinical laboratories at all levels of hospitals and can be used in conjunction with their automated systems.

Precautions for using RSR AQP4Ab kit:
Before testing, check various instruments, turn on the enzyme-linked immunosorbent assay reader, and stabilize for about 20 minutes.
2. The test sample should be clear and free of impurities, otherwise it will directly affect the results.
Before the experiment begins, the reagent kit should be taken out and left at room temperature for 30-40 minutes to allow all reagents to return to room temperature, in order to make the detection results more stable.
4. Try to conduct double standard experiments as much as possible to ensure the accuracy of the data.
5. The substrate is toxic and the termination solution is corrosive to the skin, so contact should be avoided.
In the experiment, the substrate should be stored away from light. The enzyme-linked immunosorbent assay (ELISA) plate can be disassembled, and any excess parts should be sealed and stored at low temperature.
