Enterovirus is one of the main pathogens causing hand, foot and mouth disease in infants and young children. Human enterovirus 71 was isolated from fecal samples of infants with central nervous system disorders in California in 1969. 1. What is enterovirus?
Enteroviruses belong to the family of small RNA viruses, which is a large family containing multiple viruses, including:
Coxsackievirus (divided into Group A and Group B)

Enterovirus 71 (EV71) and Coxsackievirus A16 (CVA16) (often causing hand, foot, and mouth disease)
These viruses are mainly transmitted through fecal oral route and can also be transmitted through respiratory droplets.
2. Types and significance of antibodies
In medical testing, two main types of antibodies are usually detected, which represent different stages of infection:
IgM antibody (immunoglobulin M):
Appearance time: The earliest appearance after infection.
Meaning: If a positive IgM antibody against enterovirus is detected, it usually indicates a recent infection or acute phase infection. This is an important basis for diagnosing current illnesses.
Duration: The duration is relatively short, usually disappearing within a few months.
IgG antibody (immunoglobulin G):
Appearance time: appeared relatively late.

If IgG is positive and IgM is negative, it usually indicates a previous infection and the body has developed immunity.
If the IgG titer detected during the recovery period is four times or more higher than that during the acute period, it can also be diagnosed as a recent infection.
Duration: Long lasting, even potentially lifelong, providing long-term protection.
Why do we need to test?
Doctors usually recommend testing in the following situations:
Suspected hand, foot, and mouth disease or herpetic pharyngitis: especially when the symptoms are atypical, or when it is necessary to distinguish whether it is caused by EV71 or other viruses.
Aseptic meningitis or encephalitis: Enterovirus is a common cause of viral meningitis.
Myocarditis or pericarditis: certain enteroviruses may invade the heart.
Severe infection in newborns: Newborns infected with enteroviruses may be in critical condition.
4. Precautions for interpreting test results
Cross reactivity: There are many types of enteroviruses, and cross reactivity may exist between different types, leading to false positives in test results or difficulty in determining which specific virus it is.
Window period: In the very early stage of infection, antibodies may not have been produced yet, and the test may be negative at this time, but the patient is indeed infected. At this time, it is usually necessary to combine nucleic acid testing (PCR) for diagnosis, as nucleic acid testing has higher sensitivity in the early stages of the disease.
The impact of vaccination: Currently, vaccines are mainly available for poliovirus and EV71. People who have received relevant vaccines will also have corresponding IgG antibodies in their bodies, which is a normal immune response and does not mean they are sick.
