Chapter 5 Requirements for Test Management System Standards:
5.1 Management System for Construction Site Laboratory

5.2 Equipment Management System (including equipment calibration, use, and maintenance)
5.2.1 When purchasing test instruments and equipment, first prepare the purchase plan and put forward the purchase application. When purchasing instruments and equipment, pay attention to the progressiveness of performance and the reliability of quality, as well as whether the range and accuracy meet the test requirements; For precision and valuable equipment, the manufacturer's ISO certificate and the factory measurement verification certificate or test report of the instrument and equipment should be checked.
5.2.2 Instruments and equipment should be kept by dedicated personnel, and a management ledger and file for instruments and equipment should be established.
5.2.3 Main and commonly used instrument and equipment operating procedures shall be posted on the wall, and a "Instrument and Equipment Use and Maintenance Record Book" shall be provided. After use or maintenance, it shall be carefully filled out.
5.2.4 Carry out metrological calibration and self inspection and self calibration of experimental instruments and equipment, ensure that the instruments and equipment are in good working condition, the accuracy meets the testing requirements, and keep relevant records and other materials.

Electronic instruments that are not used for a long time should be powered on every three months, with each power on lasting no less than half an hour.
5.2.10 The scrapping of instruments and equipment should be applied for by the equipment administrator, approved by the supervisor, and processed according to the prescribed procedures. For equipment calibration, operating procedures, usage, and maintenance, please refer to the "Instrument and Equipment Self Calibration Regulations" and "Operation Manual".
5.3 Sample Management System: Standard Requirements for Site Laboratory Test Management System
5.3.1 Sampling personnel or external commissioned personnel should fill out a test commission form when sending samples to the laboratory. When receiving samples, the sample administrator should check the status of the samples, such as appearance, packaging, specifications, models, etc., and count the samples. They should carefully check the integrity of the samples and their attachments, check whether the properties and status of the samples are suitable for the tested items, confirm that they are correct, and label them according to the prescribed number.
5.3.2 When the tester receives the sample, they should inspect and accept the sealing status, sampling quantity, specifications and models of the sample according to the testing standards. Samples that do not meet the requirements are not allowed to be used for testing, and the sampler is required to supplement or resample them.
Samples that need to be processed before the experiment should be processed and prepared strictly according to standard methods. When handling the sample, prevent damage or destruction to avoid affecting the test results.
5.3.4 The environment and facilities of the sample room should meet the storage conditions of the samples, such as temperature, humidity, waterproofing, fire prevention, anti-theft requirements, etc.
5.3.5 There should be a dedicated person in charge of the sample room, who should classify and store the samples, clearly label them, keep them in the account, investigate the cause of sample loss or confusion, and handle them as quality accidents.
5.3.6 For samples that have been inspected, the testing personnel should write the words "inspected" and the testing time on the sample identification card or its packaging bag.
5.3.7 The retention of samples shall be processed with the consent of the technical director or laboratory director in accordance with the prescribed requirements; Destructive testing items generally do not retain samples after testing, unless there are regulations or other special requirements for inspection. Samples retained for non-destructive testing projects can be processed if there are no abnormalities after 7 days from the issuance of the testing report. After testing and inspection, samples of cement, admixtures, and other materials need to be retained for storage, with a storage period generally not less than one month.
5.4 Management System for Test and Inspection Records
During the testing process, the testing personnel should carefully keep relevant test records, ensuring that the data is true, accurate, and not arbitrarily altered or supplemented afterwards.
5.4.2 The original records shall be filled in according to the required format and content, numbered according to a unified numbering rule, and the recorded content shall be complete and legible. Carbon pencils or signature pens shall be used for writing, and pencils or red pens shall not be used.
5.4.3 Original records shall not be altered arbitrarily. If alterations are necessary, they shall be carried out in accordance with the methods specified in the Quality Record Control Procedure. The blank space of the original record should be marked with a "/" to indicate the completeness of the data, and the significant figures of the original record should strictly follow the regulations.
5.4.4 The model, number, range, etc. of the main instruments used should be recorded. For testing items with temperature and humidity requirements, the temperature and humidity during the testing process should be recorded.
5.4.5 The original records should indicate the standard number used during the testing and inspection, and must be signed by the testing, calculation, and review personnel, without omission or proxy signature.
5.4.6 Original records must be reviewed, and the main contents of the review include:
5.4.6.1 Whether the adopted testing and evaluation standards are correct, and whether the selected instrument accuracy or range is appropriate;
5.4.6.2 Whether the recorded data is accurate, whether the calculated data is correct, and whether the retention of significant figures and rounding of numbers comply with specifications;
5.4.6.3 Whether the use of legal units of measurement is correct;
5.4.6.4 Whether the language used is accurate and standardized;
5.4.6.5 Fill in the project, signature, and date to ensure completeness.
5.4.7 After the completion of the original records, a test report should be promptly issued and submitted to the data officer for archiving and management, without affecting the timeliness of the test and detection.
5.4.8 If there is a power outage or intentional use of instruments and equipment during the testing process, it is necessary to report to the laboratory manager in a timely manner, conduct maintenance and retest, and indicate it in the test record.
5.4.9 The test and inspection reports, along with the original records, shall be kept by the archive room.
5.5 Management System for Review and Issuance of Test and Inspection Reports
5.5.1 The test and inspection report is the main basis for engineering quality assessment and also the data for project completion acceptance. The test and inspection report should be accurate, clear, explicit, and objective.
5.5.2 The report shall be filled out in accordance with the prescribed form and numbered uniformly.
5.5.3 The evaluation criteria should be sufficient, and the conclusions should be concise, accurate, and clear in text; For blank spaces in projects that are not involved, use a "/" to cross them out. Printed test reports of various types must not be manually altered.
5.5.4 The report must strictly follow the signature procedures, with separate signatures for testing, review, and audit, and proxy signatures are strictly prohibited. The report must be stamped with the inspection seal of the on-site laboratory test report before it can take effect.
5.5.5 Special materials and engineering entity inspection reports should be accompanied by a cover and necessary explanations, and stamped with a saddle seal.
5.5.6 The report should be submitted in a timely manner. Except for projects with special requirements for the test cycle, general project laboratories at all levels should issue the report within 48 hours after accepting the commission.
5.5.7 Establish a report distribution registration ledger, specifying the report name, number of copies distributed, sender, recipient, and date of collection. The sender and recipient must sign and confirm.
5.6 Archive Management System:
5.6.1 The data officer is responsible for the management of standards, specifications, procedures, various documents, and materials. They shall classify and store them according to requirements, and each data box shall have an internal catalog and be clearly labeled;
5.6.2 The original records and test inspection reports formed during the testing process shall be signed and submitted by the testing personnel to the data officer for sorting and archiving;
5.6.3 Designate a dedicated person to regularly backup electronic records. In addition to backing them up on computer hard drives, they should also be regularly backed up on CDs or portable hard drives, and classified, clearly labeled, and stored properly to prevent loss or damage;
5.6.4 For invalid and invalidated documents, the data officer shall promptly retrieve them according to the original distribution scope and location, and fill out the "Document Receipt and Distribution Registration Form" to prevent misuse. Properly handle invalidated test and inspection reports to prevent leakage;
5.6.5 The borrowing or retrieval of experimental data must be approved by the laboratory director before going to the data administrator to handle the retrieval procedures;
5.6.6 Various management documents should be reviewed by the laboratory director and implemented after signing opinions;
5.6.7 Keep confidential the original records, reports, and product technical documents and drawings provided by users for testing and inspection, and set up a dedicated counter to store important data and documents.
5.7 Management System for Outsourced Test Sampling and Delivery
5.7.1 When conducting outsourced testing outside the laboratory, the qualifications, testing capabilities, credit level, service quality, and fee standards of the outsourced testing and inspection agency must be examined and approved by the supervisor and owner before implementation.
5.7.2 For entrusted testing and inspection institutions that do not meet qualifications, have low abilities, engage in fraud, and charge seriously unreasonable fees, once discovered, the entrusted testing must be immediately stopped and reported internally.
5.7.3 The laboratory must establish an outsourced testing plan and ledger, and the outsourced testing reports must be managed separately.
5.7.4 The technical and quality managers of the laboratory shall oversee the technical and quality issues of externally commissioned tests, review the reports of externally commissioned tests, and promptly resolve any issues.
5.7.5 The test methods and evaluation criteria used for all outsourced tests must comply with the corresponding technical standard requirements. At least two copies of the outsourced test report shall be provided.
5.7.6 For outsourced testing projects, the supervising personnel must witness the sample delivery, complete the sample sealing and labeling work, and obtain the confirmation certificate of sample receipt from the commissioned laboratory.
5.7.7 For important experimental projects, commission testing to retain samples for retesting.
5.7.8 It is strictly prohibited for the supervision unit and the construction unit's laboratory to entrust the same testing project to the same testing and inspection agency for testing. For the testing projects of the testing agency, the supervision unit shall witness the submission of samples.
5.8 Non conforming Product Management System
5.8.1 When non-conforming phenomena are found during the inspection of accidents, the reasons for the non-conforming must be carefully analyzed to prevent unexpected use, installation, or use of non-conforming products in the projects undertaken by our company, in order to ensure the quality of the project.
5.8.2 When the test results are found to be unqualified, the testing personnel should immediately report to the technical manager of the laboratory, notify the construction manager, stop the continued construction of the part of the project, and make proper markings. They should wait for the results of the re inspection before formulating a handling opinion.
5.8.3 The person in charge of experimental technology shall organize the testing personnel to retest this part, and if possible, use other instruments of the same type for testing, and personally supervise all operation processes and data organization processes.
5.8.4 If the retest results are qualified, the person in charge shall convene the personnel participating in the test to understand and analyze the possible reasons for the occurrence of * nonconformities, and analyze whether it is caused by testing operation errors, instrument inaccuracies, data organization errors, etc. If necessary, conduct another repeat experiment. When it is determined that the unqualified phenomenon is caused by the fault of the testing work itself, the laboratory shall issue a written explanation report and notify the relevant construction technical responsible person that normal construction can be carried out. The laboratory responsible person must take effective measures to strengthen personnel education, strictly abide by operating procedures, and calibrate instruments and equipment as required to prevent similar situations from happening again.
5.8.5 If the re inspection is not qualified, the part shall be determined as unqualified. The laboratory issues a written report and reports it to the chief engineer for handling as non-conforming products. The chief engineer, construction technical leader, quality inspection engineer, professional engineer, testing personnel, etc. shall review the non-conforming products and determine the handling methods.
5.8.6 Non conforming products shall be corrected and prevented as required. To meet the original requirements and take measures to prevent the recurrence of the phenomenon.
5.9 Accident Analysis Report System for Testing
5.9.1 When any non-conforming phenomena are found during the testing process, the reasons for the non-conforming products must be carefully analyzed to prevent the unexpected use, installation, or operation of non-conforming products in the project undertaken by the company, in order to ensure the quality of the work.
5.9.2 When the test results are found to be unqualified, the testing personnel shall immediately report to the technical director of the laboratory, notify the construction director, stop the continued construction of the part of the project, and make proper markings. They shall wait for the results of the re inspection before deciding how to handle it.
5.9.3 The technical director of the laboratory shall organize the testing personnel to retest this part, and if possible, use other instruments of the same type for testing, and personally supervise all operation processes and data organization processes.
5.9.4 If the re inspection result is qualified, the technical director of the laboratory shall convene the personnel participating in the test to understand and analyze the possible reasons for the occurrence of the unqualified phenomenon, and analyze whether it is caused by testing operation errors, instrument inaccuracies, data organization errors, etc. If necessary, conduct another repeat experiment. When it is determined that the unqualified phenomenon is caused by the fault of the testing work itself, the laboratory shall issue a written explanation report and notify the relevant construction technical responsible person that normal construction can be carried out. 5.9.5 If the re inspection fails and the part is determined to be non-conforming, the laboratory shall issue a written report and report to the chief engineer for handling as non-conforming. The chief engineer, construction technical leader, quality inspection engineer, professional engineer, laboratory personnel, etc. shall review the non-conforming products and determine the disposal methods.
5.9.6 Non conforming products shall be corrected and prevented as required. To meet the original requirements and take measures to prevent the recurrence of the phenomenon.
5.9 Laboratory Safety and Environmental Management System:
5.9.1 The laboratory should establish systems for fire prevention, gas prevention, and safe use of electricity, strengthen safety education for personnel, regularly inspect safety operations, promptly eliminate hidden dangers, and prevent safety accidents from occurring.
5.9.2 The testing workshop should be equipped with fire-fighting equipment, which should be kept in good condition. All personnel should be familiar with the use of fire-fighting equipment. Flammable and explosive dangerous goods, as well as highly toxic and radioactive hazardous materials, should not be piled up or placed randomly. Isolation and protection measures should be taken, and a dedicated person should properly store them.
5.9.3 Electrical equipment must be kept intact and undamaged, and no one is allowed to add or move electrical circuits or increase loads without authorization. Regularly inspect electrical equipment by designated personnel, and ensure the normal operation of electrical equipment in accordance with electricity regulations.
5.9.4 Consciously abide by safety regulations and relevant rules, strictly follow operating procedures, and shall not engage in illegal operations; Immediately cut off the power, heat source, and water source after use.
5.9.5 High temperature heating test shall strictly follow the requirements of the regulations for heating and cooling, and shall not be operated in violation of regulations. During the test period, operators shall not leave the operating room.
Personnel unrelated to the experiment are not allowed to enter the laboratory without permission. Test items that are prone to specimen splashing are strictly prohibited from standing next to the machinery. Mechanical equipment such as presses and material testing machines should be equipped with metal protective covers or safety nets. When using protective nets, the mesh size should not exceed 1cm x 1cm.
5.9.7 If there are any abnormal conditions in the instrument and equipment during the test process, the machine should be stopped immediately for inspection and treatment before resuming work, and this should be noted in the "Instrument and Equipment Use and Maintenance Record".
5.9.8 Carry out anti-theft measures, strengthen the storage of office computers, files, and materials, close all doors and windows after work, and prevent the loss of instruments, equipment, and important test data.
5.9.9 In accordance with the requirements of the quality, environment, and occupational health and safety system documents, conscientiously carry out environmental management work in the workplace, office, and living areas, especially the treatment of experimental waste and chemical waste liquids should be handled in accordance with relevant regulations.
5.10 Fire Emergency Management System

6.5 Management of Testing Work
6.5.1 Management of Standards, Specifications, and Procedures
The construction site laboratory should collect complete relevant regulations, specifications, and standards based on the project engineering content and authorized parameters. The construction site laboratory should control the collected standards and regularly establish a list of standards for the laboratory. If regulations, specifications, and standards expire and become invalid, the laboratory should promptly obtain valid versions for replacement, and notify laboratory personnel and relevant departments in writing to stop using them. 6.5.3.2 The conclusion of the test report should be expressed in a standardized and accurate manner.
6.5.3.3 The construction unit's on-site laboratory shall establish a self inspection test ledger, and the third-party on-site laboratory shall establish a sampling test ledger corresponding to the self inspection ledger of the construction unit's on-site laboratory.
