Extracorporeal Circulation Kit: Composition, Clinical Application, and Standardized Management

The Extracorporeal Circulation Kit (ECC Kit) is a standardized and integrated consumable combination for life support treatments such as extracorporeal circulation (ECC) and extracorporeal membrane oxygenation (ECMO) in the fields of cardiovascular surgery and critical care medicine. It integrates the core tubing, intubation, puncture accessories, and auxiliary components required for extracorporeal circulation, aiming to simplify the preoperative preparation process, ensure operational standardization, reduce the risk of cross infection, and provide safe and efficient pathway guarantees for cardiac surgery and critical patients' life support. As one of the three high-risk medical devices, its component matching, material properties, and sterility standards are directly related to the stability of extracorporeal circulation and patient treatment outcomes, making it the core consumable set for modern cardiovascular medicine and critical care.

1、 Core Definition and Design Concept of Extracorporeal Circulation Package

The extracorporeal circulation kit is a specialized consumable set that integrates and aseptically encapsulates the "tubing intubation puncture auxiliary accessories" required during extracorporeal circulation based on clinical standardized operation requirements. It can be divided into different specifications and models according to clinical scenarios (conventional cardiac surgery, minimally invasive cardiac surgery, ECMO support, emergency rescue), suitable for different populations such as adults, children, infants, and young children.

The core design concept revolves around three main points: standardization, unified component specifications and connection methods, and compatibility with mainstream artificial heart and lung machines and oxygenators, reducing the time required for preoperative component matching; The second is safety. All components have undergone strict aseptic treatment and biocompatibility testing to avoid risks such as blood leakage, gas embolism, and thrombosis caused by component incompatibility; The third is convenience, as there is no need to prepare various scattered consumables separately before surgery. They can be used immediately after opening the box, shortening the preoperative preparation time, especially suitable for emergency rescue scenarios.

Maike Wei extracorporeal circulation kit BE-PLS 20502、 Core components of extracorporeal circulation kit

The component configuration of the extracorporeal circulation kit needs to meet the actual clinical needs. The kit components from different manufacturers (such as Medtronic, Tylenol, Bard) may vary slightly, but the core composition is the same, mainly divided into five categories. Some kits may add special functional components to adapt to complex surgical scenes.

(1) Core pipeline components

As the "blood flow channel" of extracorporeal circulation, the tubing is the core of the package and is made of medical grade highly biocompatible materials. The inner wall is smooth, resistant to twisting and collapse, and can withstand high pressure and high-speed blood flow during extracorporeal circulation, reducing hemolysis and thrombus formation.

•

Arterial infusion tubing: connects the oxygenator and arterial cannula, responsible for infusing oxygenated arterial blood back into the patient's body. The tubing has a thick wall and strong pressure resistance, and some parts have pressure monitoring interfaces for real-time monitoring of infusion pressure during surgery.

•

Venous drainage tubing: connects the venous catheter to the oxygenator, responsible for draining the patient's systemic venous blood to the oxygenator. The tubing has a thick diameter and low resistance, ensuring sufficient return blood volume and avoiding high venous pressure.

•

Auxiliary tubing: including pump tubing (compatible with roller pumps/centrifugal pumps), blood storage tank connection tubing, pressure monitoring tubing, exhaust tubing, and some kits include retrograde perfusion tubing and left heart drainage tubing, suitable for complex cardiac surgery needs.

(2) Extracorporeal circulation catheterization component

Intubation is the key to connecting the patient's blood vessels with the extracorporeal circulation tubing. The catheters inside the package are all specialized for extracorporeal circulation, classified by function and location, and suitable for different surgical scenarios. The materials are mostly medical polyurethane (PU) and soft PVC, and some products have heparin coating to reduce the risk of thrombosis.

•

Arterial catheterization: commonly used ascending aorta catheterization and femoral artery catheterization, with a round and blunt design and multiple side holes to reduce vascular intimal injury, with specifications ranging from 14Fr to 28Fr (commonly used in adults ranging from 20-24Fr).

•

Venous catheterization: divided into single-stage venous catheterization (separate drainage of the superior/inferior vena cava), two-stage venous catheterization (simultaneous drainage of the superior and inferior vena cava), large caliber, low resistance, suitable for long-term extracorporeal circulation.

•

Specialized intubation: Some kits include left heart drainage intubation and coronary sinus catheterization (for retrograde perfusion), suitable for myocardial protection and complex intracardiac malformation surgery.

(3) Puncture auxiliary accessory

Mainly used for minimally invasive extracorporeal circulation, ECMO catheterization, or preoperative establishment of backup vascular access, Seldinger puncture technology is adopted, with complete components to avoid specification mismatch caused by separate preparation.

•

Puncture kit: includes arterial/venous puncture needle (14G-18G, large caliber thin-walled), J-shaped guide wire, and step-by-step dilator, which facilitates precise puncture of blood vessels, dilates the entrance, and reduces vascular tearing.

•

Vascular sheath/introducer: designed with a hemostatic valve to prevent bleeding and air from entering after puncture, suitable for minimally invasive catheterization scenarios such as femoral artery and vein minimally invasive catheterization.

(4) Fixed and connected components

Ensure the stability of the tubing and intubation during extracorporeal circulation, avoiding displacement, detachment, and air leakage. The core components include:

•

Connection connector: standard Ruhr connector, quick connector, seamlessly matched with tubing, intubation, and artificial heart lung machine, pressure resistant, leak proof, and anti detachment.

•

Fixed accessories: suture ring, fixing plate, ligature tape, sterile transparent dressing, used to fix intubation and tubing to avoid displacement caused by intraoperative pulling.

•

Auxiliary tools: vascular forceps, blocking forceps, used for freeing blood vessels, controlling blood flow, and facilitating intubation and fixation.

(5) Aseptic and auxiliary components

•

Aseptic consumables: sterile gloves, iodine disinfectant cotton pads, sterile drapes, some bags containing heparin saline syringes for preoperative flushing and anticoagulation.

•

Exhaust components: exhaust needle, exhaust bag, used to exhaust air during pre flushing of extracorporeal circulation pipelines to avoid the risk of air embolism.

3、 Classification and clinical adaptation scenarios of extracorporeal circulation kits

Extracorporeal circulation kits can be divided into three categories based on clinical use and applicable population. The component configuration of each category of kit is highly targeted to meet the needs of different scenarios, avoiding waste of consumables and functional redundancy.

(1) Specialized set for routine cardiac surgery

Suitable for routine extracorporeal circulation surgeries such as coronary artery bypass grafting, aortic valve replacement, and simple congenital heart surgery, the core features are complete components and wide adaptability, including two-stage venous catheterization, ascending aortic catheterization, complete piping system, and basic puncture accessories. The adult and pediatric models are complete, which can meet the extracorporeal circulation needs of most cardiac surgeries.

(2) Minimally invasive cardiac surgery/EMCO specialized kit

Adapting to minimally invasive cardiac surgery (such as thoracoscopic cardiac surgery) and ECMO life support, the core feature is minimally invasive and refined components, including long handled femoral arteriovenous catheterization, minimally invasive puncture kit, and miniaturized tubing. The tubing is soft and easy to bend, adapting to minimally invasive incisions and position limitations, reducing surgical trauma; The ECMO special kit will also add membrane lung connection tubing and flow monitoring interfaces to meet the long-term support needs of ECMO.

(3) Emergency rescue kit

Suitable for emergency scenarios such as acute myocardial infarction, acute heart failure, and severe respiratory failure, the core features are out of the box use, easy operation, simplified components but complete core functions, including quick puncture kit, large caliber intubation, short tube route, which can quickly establish extracorporeal circulation/ECMO pathway, and save time for emergency patients.
